Application Process:

Your medical need information is personal and we respect that - no one has the right or authority to force you to disclose your medical information. In order to qualify your service dog for registration you must attest that you do have a medical need and that your dog has been appropriately trained and is not a public nuisance. This information will remain completely confidential.

Please print/save to your hard drive and complete the application form HERE or in Plain Text email or post complete application along with a clear photograph of your service dog and payment of $42.50 by check, money order or by PayPal USPS address is 930 Rosedale Ave. #36, Capitola, CA 95010 - Checks and money orders may take up to 10 days to process, PayPal orders will be processed within two business days.

Most counties require you to clean up after your dog - we encourage this and request that either you, or another person be prepared to immediately clean up after your service dog.
